Blue Cross of California: CaliforniaCare
CaliforniaCare is a Health Maintenance Organization (HMO) using a network of doctors and hospitals that have contracted with Blue Cross to fulfill your healthcare needs.
CaliforniaCare provides comprehensive healthcare treatment without the forms and out-of-pocket expenses of conventional medical programs.
Under this plan, you choose a Primary Care Physician from a Medical Group or Independent Practice Association in the CaliforniaCare directory. The Primary Care Physician you choose will be the doctor responsible for your overall healthcare and is the doctor you must call first when you need medical treatment. If you require a specialist, your Primary Care Physician will provide a referral for you.
As a CaliforniaCare member, you pay no deductible and you don't fill out any claim forms. You will pay a $10 co-payment each time you see the doctor.
Blue Cross of California: Prudent Buyer Plan or Blue Card (Outside of CA plan)
Prudent Buyer is a Preferred Provider Organization (PPO) health plan with a large group of Participating Providers. Each time you wish to see a doctor, you may visit any Provider in the Prudent Buyer directory and you may change doctors at any time.
Blue Cross pays 80% of your treatment cost -- once you satisfy a $300 annual individual deductible ($600 annual family deductible). After Blue Cross has made $4000 in total payments for covered expense each member incurs during a calendar year, the member's co insurance for the remainder of the year will be at no charge for PPO Providers.
Under this Plan you may use a Non-participating Provider, however, Prudent Buyer pays only 70% of the Customary and Reasonable charges for your treatment (after your deductible) and the total out-of-pocket maximum cost increases to approximately $1,715 per individual.
Kaiser Permanente
The Kaiser Permanente HMO is an all-in-one, one stop shop for your medical needs including doctor's office visits, urgent care, prescriptions, specialty care, hospitalization, etc. Benefit levels match the Blue Cross HMO; however, he contribution costs are significantly less. $10 office co-pay. Must live in a Kaiser service area.
Dental Benefits
MetLife Preferred Dentist Plan (PDP)
The MetLife PDP lets you use any licensed provider you chose, but allows you to save money if you use a PDP dentist. PDP dentists agree to charge you a plan-negotiated fee and you pay only your percentage share of this amount. For non-PDP dentist, the plan pays benefits based on the Reasonable and Customary (R&C) amount. You pay your percentage share of this amount plus any amounts the provider charges above the R&C. Over 60,000 credentialed dentists participate in the PDP network nationwide, which includes 12,000 specialists.
Medical and Dental Contributions
Pre-Tax Contribution Plan
Each year you choose medical and dental coverage to meet your needs. The pre-tax contribution plan allows your premium contribution to be deducted from your paycheck prior to tax withholding. All employees are automatically enrolled. If you wish to decline participation in this plan, you must complete a decline form.
Vision Benefits
VSP: Vision Service Plan
VSP provides an extensive network of vision providers under the plan. You may access providers either via the internet at www.vsp.com or by calling (800) VSP-7195.

Section 125 Enhancement Program - "Flexible Spending Accounts"
Plantronics has established an I.R.S. qualified reimbursement plan for healthcare and dependent care expense. Employees who enroll in this plan establish an account for pre-tax contributions available to cover eligible benefit expenses for the year. This is a "use it" or "lose it" account, however, and funds must be used or are forfeited at year-end. Eligible employees can become a participant on the first day of the month following their date of hire or during the annual open enrollment period.
Employee Assistance Plan
The EAP is designed to provide supplemental and confidential resources for associates who may need assistance and/or counseling for personal concerns, including but not limited to, health, family, financial, alcohol, drug, or other emotional needs. This benefit is free to all associates.
Life Insurance & Income Protection
Life Insurance and Accidental Death and Dismemberment Insurance
Should you die while employed by Plantronics, the Company provides life insurance benefits equal to 2 times your annual base salary -- up to $250,000. In the event your death is accidental, an additional 2 times your annual base salary will be paid -- up to the $250,000 maximum. Benefits under the Dismemberment plan are paid based on the specific loss. Plantronics pays all premium costs associated with this coverage.
You may purchase additional life insurance coverage for as much as 4 times your base salary, up to the $750,000 maximum. You may also purchase coverage for your spouse up to 50% of your total eligible life insurance coverage.
Additional accidental death and dismemberment insurance may be purchased for you or for you and your dependent. Coverage starts at $10,000 and has maximum of $500,000 or 10 times your annual base salary.
Business Travel and Accident Insurance
If you travel on Plantronics business, you are covered for up to $250,000 for accidental death during the time of travel. A portion of this amount would be paid for any injuries incurred while on business travel. Plantronics pays all premium costs for this plan.
Short Term Disability
Both exempt and non-exempt associates are eligible for up to 60% (weekly maximum of $2,500) of compensation (base applicable on the 8th consecutive day for any non-work related injury or illness including pregnancy).
Non-exempt employees may also use accrued sick time for disability leave. Additionally, exempt employees are eligible for a salary supplement to provide 100% of base salary for the first 10 days of a disability and 80% of base salary for up to 90 days of short-term disability leave.
Long Term Disability
The Long Term Disability benefit replaces 60% of your gross monthly earnings, less income you may receive from certain other sources (e.g., Social Security, Workers’ Compensations, etc.) The maximum monthly benefit is $12,000 if you qualify.
